Reviewed a case involving a hospitalist prescribing a patient with a history of substance abuse and obstructive sleep apnea a large quantity of opioid medication and a benzodiazepine. The patient overdosed on the prescription opioid and benzodiazepine the day after discharge and died.
Reviewed a case involving a patient with type 1 diabetes mellitus who was voluntarily admitted to a psychiatric facility and was not given long acting insulin in a timely manner after her insulin pump was removed. The person was subsequently admitted to a hospital ICU in diabetic ketoacidosis a day and a half after being in the psychiatric facility.
Reviewed a case involving a patient who had a thoracentesis in the hospital after a coronary artery bypass graft surgery. The patient subsequently developed a hemothorax, went into respiratory failure and died.
Reviewed a case involving a person who underwent a heller myotomy, toupet fundoplication and esophageal diverticulectomy. The patient developed gastroparesis post-operatively requiring a long term feeding tube. The patient also had an enterotomy complication which caused septic shock and required repeat surgery.
Reviewed a case involving a patient who developed an ileus after having an intracranial meningioma resected. The ileus developed into a colon obstruction, which led to a colon perforation and septic shock requiring multiple additional bowel resection surgeries for ischemic colitis and gangrene of small and large bowel.
Reviewed a case involving a 4 cm soft tissue mass found on a leg venous doppler. The mass was not worked up for over 18 months. The patient was subsequently diagnosed with stage 4 sarcoma and died from this.
Reviewed a case involving a person who developed a renal abscess after a foley catheter insertion during pregnancy. The person had to be readmitted to the hospital and receive a prolonged course of IV antibiotics through a PICC line due to this.
